Description

Why do intelligent, trained, and well-intentioned people make unsafe decisions in high-stakes environments?

mindFly: Follies, Realities & Human Factors explores aviation safety through the invisible forces that shape human behaviour: attention, fatigue, culture, conformity, cognitive bias, emotional pressure, organisational drift, and the fragile nature of situational awareness.

Drawing on real aviation accidents and incidents, Captain Amit Singh FRAeS examines how safety is preserved, or quietly lost, long before an accident occurs. From unstable approaches and wrong-runway alignments to fatigue, automation dependence, Crew Resource Management, accountability, and artificial intelligence, this book shows that aviation failures are rarely caused by a single error. More often, they emerge from complex interactions between people, systems, procedures, culture, and pressure.

What makes mindFly distinctive is its integration of modern human factors science with philosophical insight. Indian concepts such as awareness, restraint, salience, and disciplined action are translated into practical safety lessons for a global readership. The result is a deeply human account of aviation safety, one that treats safety not merely as compliance, but as a living organisational conscience.

Written for pilots, aviation professionals, safety leaders, regulators, trainers, students, and general readers interested in human performance, mindFly offers a compelling journey into the psychology of error and the discipline of prevention. Its lessons extend beyond aviation to healthcare, corporate leadership, emergency response, technology, and every environment where decisions carry consequences.

This is a book about flight, but more importantly, it is a book about the human mind: how it focuses, how it fails, how it conforms, how it recovers, and how it can be trained to protect life.

About the Author

Captain Amit Singh FRAeS is an aviation professional, safety specialist, and human-factors practitioner with over three decades of experience in commercial aviation and over 18,000 hours of flying experience. He has flown and commanded aircraft, including the Airbus A320 and Boeing 777, and has held senior leadership positions in flight operations, training, and aviation safety.

A Fellow of the Royal Aeronautical Society, he has served in roles including Chief of Safety, Director of Flight Operations, and Chief Pilot Training. His work focuses on human factors, pilot cognition, fatigue, organisational culture, training, and the systemic conditions that influence decision-making in safety-critical environments.

He is the Founder-Director of the Safety Matters Foundation, an organisation dedicated to evidence-led safety, human performance, and the principle that meaningful improvement comes from understanding systems rather than simply blaming individuals.

Beyond aviation, Amit has competed as a national-level target shooter—an experience that strengthened his interest in attention, mental discipline, and performance under pressure.

In mindFly: Follies, Realities & Human Factors in Aviation, he brings together operational experience, accident analysis, psychology, neuroscience, and philosophy to examine a deceptively simple question: why do capable people and sophisticated organisations sometimes make decisions they already know are unsafe?
