Health tracker is a simple book that can be used by everyone to capture the important details of their health. All the aspects of our health are covered under the various sections in the book. There are separate sections for noting down the health issues faced and their dates, the treatments undertaken for the same, doctor visits and medicines taken to resolve the issue etc. This will help users to consolidate all the health information in one place!

Basic Info
The first section is Basic info where users can fill their name, date of birth, blood group, type of diet (vegetarian or non-vegetarian or vegan), details of habits like smoking and drinking, parents and siblings blood groups, spouse and children blood groups, health history to note if they have diabetes or heart issues or any other issues, health history of parents and siblings etc.
Health history
The second section is Health history where users can give detailed information of major health issues faced and the date and time period of the issue. Life style diseases like diabetes, heart issues, thyroid issues etc. can be noted and the date on which the issue started can be noted down in this section.
Health issues
The third section is health issues where users can note down the date and details of various health issues faced and the treatment undertaken for the same. This section can give a complete overview of the various health issues we have faced!
Treatments taken
In this section, users can note down the date and details of various treatments undertaken to resolve health issues.
Medicines taken
In this section, users can note down the details of medicines taken, date, purpose and dosage of the medicines.
Doctor visits
Users can note down the date and details of doctor visits in this section,
Test Reports
Users can note down the date and details of the various medical tests and their reports in this section,
Calendars
This section has yearly calendars (without any week days mentioned) and box calendars. This will enable the users to use the calendars for any year. They can mention the year name and calendar name at the top and start using the calendars. Fill the calendar index with the calendar names for easy navigation. The yearly calendars can be used to note down health issues, doctor visits, Hba1c values etc. The box calendars can be used to note and count the number of times an activity was done or an event occurred. Example are symptom tracking for health issues, women’s period tracking, drinking and smoking habits tracking, fasting or diet tracking etc.
There are six yearly calendars ( 2025 to 2030 ) with weekdays mentioned which can be referred in case the user wants to find out the day of the week. These calendars can also be used to note down the health issues faced during that year and thus can capture all the details of the health issues faced for six years with just one tracker. Users can refer the examples for a better understanding of how to use the tracker!
This tracker can capture 5 to 6 years of health data of a person’s life. After six years, we can get a new tracker and continue the notes. This book can help us to record all the important information of our health in one place and thus access the information whenever we need and enable us to understand and manage our health better! This book will be useful for everyone to manage their health!
